The Social Worker will play a key role in the success of our community which includes admissions and resident case management for our 44-private room skilled nursing residents. The Social Worker will be responsible for troubleshooting and resolving issues to maximize resident quality of life and person-centered care. This significant position offers tremendous opportunity for the right person.

Responsibilities include:

* Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.

*Contributes to team effort by accomplishing related results as needed.

* Handles preadmission and admission interviews, informing resident and family of available services, resident rights and responsibilities.

*Helps residents by assessing their situation; obtaining required services.

*Handles discharge planning and provides assistance to residents and families on request.

* Determines nature of resident's situation by interviewing the resident; assessing medical, psychological, emotional, and social information; making one on one visits.

* Establishes course of action by exploring options; setting goals with resident.

* Obtains assistance for resident by referring him/her to provider resources; arranging for appointments; establishing rapport with other provider.

* Maintains record of residents by documenting and MDS.

* Monitors residents by periodic follow-up.

* Maintains operations by following policies and procedures; participating in quality reviews; reporting needed changes.

* Complies with federal, state, and local legal requirements by studying existing and new legislation; enforcing adherence to requirements; advising management on needed actions.

* Maintains resident confidence and protects operations by keeping information confidential.

* Contributes to team effort by accomplishing related results as needed. Provides staff education and development as needed on current resident needs.

Skills/Qualifications:

Requirements:

  • Previous experience in a geriatric setting
  • Excellent organizational, time management, and interpersonal skills
  • Ability to advocate for resident’s rights and needs
  • MSW
  • MDS proficient
  • MS Office proficient
